RESUMO

La anemia perioperatoria constituye un factor independiente de riesgo de morbimortalidad posoperatoria. Sin embargo, persisten barreras conceptuales, logísticas y administrativas que dificultan la implementación generalizada de protocolos para su manejo. El coordinador del proyecto convocó a un grupo multidisciplinar de ocho profesionales para elaborar un documento de consenso sobre el manejo de la anemia perioperatoria, con base a en serie puntos claves (PCs) relativos a su prevalencia, consecuencias, diagnóstico y tratamiento. Estos PCs fueron evaluados utilizando una escala Likert de 5 puntos, desde «totalmente en desacuerdo [1]» a «totalmente de acuerdo [5]». Cada PC se consideró consensuado si recibía una puntuación de 4 o 5 por al menos siete participantes (> 75%). A partir de los 36 PCs consensuados, se construyeron algoritmos diagnóstico-terapéuticos que pueden facilitar la implementación de programas de identificación precoz y manejo adecuado de la anemia perioperatoria, adaptados a las características de las instituciones hospitalarias de nuestro país.(AU)

CONTEXT: Music listening (ML) has been shown to have a beneficial effect on patients with cancer. However, novel intervention approaches are needed. OBJECTIVES: We aimed to determine whether ML based on the iso-principle, conducted using a mobile application (GloMus), improves symptom burden, quality of life (QoL), anxiety, and depression in patients undergoing stem cell transplantation (SCT) and intensive induction chemotherapy for acute myeloid leukemia (AML). METHODS: In this randomized controlled clinical trial, we assigned 71 patients to the ML or standard care (SC) groups, stratified by the reason for admission (AML, allogeneic-SCT, or inpatient/outpatient autologous-SCT). Upon admission, participants in the ML groups were invited to undergo daily ML sessions designed to change negative moods into positive ones (iso-principle). The intervention consisted of listening to pre-recorded classical music ordered by beats per minute and tonality. Symptom burden (Edmonton Symptom Assessment System-Revised) was assessed in the ML groups before and after each session. Anxiety, depression (Hospital Anxiety and Depression Scale), and QoL (Functional Assessment of Cancer Therapy-Bone Marrow Transplantation/Leukemia) were measured weekly in the ML and SC groups. RESULTS: Symptom burden in both allogeneic- and inpatient autologous-SCT ML groups reduced after the intervention. In all experimental groups, clinically important improvements were observed after ML sessions. No differences were found between the groups (ML vs. SC) at different weeks of admission regarding anxiety, depression, and QoL. CONCLUSIONS: ML based on our innovative iso-principle strategy, conducted using GloMus, reduced the symptom burden in patients undergoing allogeneic- and inpatient autologous-SCT (ClinicalTrials.gov number, NCT05696457).

Quiescent cells require a continuous supply of proteins to maintain protein homeostasis. In fission yeast, entry into quiescence is triggered by nitrogen stress, leading to the inactivation of TORC1 and the activation of TORC2. Here, we report that the Greatwall-Endosulfine-PPA/B55 pathway connects the downregulation of TORC1 with the upregulation of TORC2, resulting in the activation of Elongator-dependent tRNA modifications essential for sustaining the translation programme during entry into quiescence. This process promotes U34 and A37 tRNA modifications at the anticodon stem loop, enhancing translation efficiency and fidelity of mRNAs enriched for AAA versus AAG lysine codons. Notably, some of these mRNAs encode inhibitors of TORC1, activators of TORC2, tRNA modifiers, and proteins necessary for telomeric and subtelomeric functions. Therefore, we propose a novel mechanism by which cells respond to nitrogen stress at the level of translation, involving a coordinated interplay between the tRNA epitranscriptome and biased codon usage.

Lentinus crinitus (L.) Fr is a wild macrofungus that is popular as antimicrobial and various biological activities. This study aims to determine the capacity growth stimulation of Lactobacillus paracasei and antimicrobial activity of aqueous extracts of L. crinitus obtained from wild basidiomata, mycelial biomass by liquid fermentation and spent mushroom substrate obtained by solid-state fermentation. The antimicrobial activity was investigated against bacterial and fungal pathogens and growth stimulation L. paracasei probiotic bacterium. The total carbohydrate and ß-glucan contents of the extracts were determined using colorimetric analysis. The aqueous extracts obtained showed inhibition against Fusarium oxysporum., Penicillium sp., Rhizopus oryzae, Aspergillus niger, Escherichia coli and Salmonella typhimurium. The aqueous extract obtained from wild basidiomata, and mycelial biomass showed the highest percentage of stimulation of L. paracasei growth in 48 h. The extracts obtained from L. crinitus have antimicrobial potential and stimulating capacity of the probiotic Lactobacillus paracasei. Additionally, different biotechnological techniques such as liquid and solid-state fermentation can be used to obtain aqueous extracts.

BACKGROUND: Attention deficit/hyperactivity disorder (ADHD) is a chronic condition with long-term associated risks. OBJECTIVE: To analyze the clinical status, occupation, and daily life difficulties of two cohorts of children diagnosed with ADHD (2004 and 2009) after five and 10 years. MATERIAL AND METHOD: Descriptive, longitudinal study using the WOMI database corresponding to Oviedo, Asturias, Spain. Children with ADHD detected by their primary care pediatrician were included. A voluntary follow-up telephone interview was requested. Outcome variables were current clinical and occupational status, ADHD symptoms according to SNAP-IV, and Strengths and Difficulties Questionnaire (SDQ). RESULTS: The interviewed sample consisted of 95 subjects, out of whom 71 answered the SNAP-IV and SDQ questionnaires; 60.70% of the sample did not receive treatment at the time of follow-up, 4.7% had no occupation, 25.40% still had ADHD global symptoms above the clinical cutoff point, and 66.2% had difficulties with moderate interference. CONCLUSIONS: ADHD symptoms decline as people grow up. Mental health comorbidities and academic dropout were not confirmed in the sample.

RESULTADOS: La muestra entrevistada estuvo formada por 95 sujetos, de los cuales 71 respondieron a los cuestionarios SNAP-IV y SDQ; 60.7 % de la muestra no recibía tratamiento en el momento del seguimiento, 4.7 % no tenía ninguna ocupación, 25.4 % mantenía síntomas globales de TDAH en nivel superior al punto de corte clínico y 66.2 % presentaba dificultades con interferencia moderada. CONCLUSIONES: Los síntomas del TDAH disminuyen conforme las personas crecen. Las comorbilidades de salud mental y el abandono académico no se confirmaron en la muestra.

PURPOSE OF REVIEW: Cerebral palsy (CP) is a group of disorders caused by non-stabilized cerebral lesions. Individuals with this disorder are at a higher risk of suffering from malnutrition and other related detrimental effects to their quality of life. For this reason, accurate methods of nutritional assessment are vital for people suffering from this condition. While assessment of nutritional status in children with CP has been extensively studied, very few studies have been carried out on adults. These limitations are due to the great anatomical-functional variability characteristic of this syndrome. Difficulties that derive from this variability in adult patients with CP mean that there remains an urgent need for certain standards of nutritional assessment for this population. The objective of this review is to compile the latest trends in nutritional assessment in adults with CP to guide the development of a conceptual framework for future research. RECENT FINDINGS: With this aim, relevant studies have been identified. The most commonly used technique to evaluate nutritional status is the BMI because of its ease-of-use. However, its well-known limitations fail to adequately estimate the nutritional status in this population, with measurements of patients with CP yielding results that are much less accurate than those that already exist in the general population. Although more studies are needed, kinanthropometry is considered one of the most reliable techniques; nevertheless, the anatomical limitation characteristic of CP plays a limiting factor.

Se presenta el caso de una mujer de 79 años con glaucoma severo y falta de adherencia al tratamiento, que es sometida a cirugía de catarata e implante XEN® en su ojo izquierdo. Dos semanas después de la intervención se objetiva una erosión conjuntival con exposición del extremo distal del implante, por lo que se realiza una reparación quirúrgica mediante la combinación de una sutura aposicional del tubo, adaptando su trayectoria a la curvatura escleral, y el recubrimiento con injerto de membrana amniótica. Tras 6 meses de seguimiento, presenta una presión intraocular controlada, sin necesidad de tratamiento adicional, y sin mostrar progresión de su enfermedad (AU)

The molecular characteristics of pediatric brain tumors have not only allowed for tumor subgrouping but have led to the introduction of novel treatment options for patients with specific tumor alterations. Therefore, an accurate histologic and molecular diagnosis is critical for optimized management of all pediatric patients with brain tumors, including central nervous system embryonal tumors. We present a case where optical genome mapping identified a ZNF532::NUTM1 fusion in a patient with a unique tumor best characterized histologically as a central nervous system embryonal tumor with rhabdoid features. Additional analyses including immunohistochemistry for NUT protein, methylation array, whole genome, and RNA-sequencing was done to confirm the presence of the fusion in the tumor. This is the first description of a pediatric patient with a ZNF532::NUTM1 fusion, yet the histology of this tumor is similar to that of adult cancers with ZNF::NUTM1 fusions reported in the literature. Although rare, the distinct pathology and underlying molecular characteristics of the ZNF532::NUTM1 tumor separates this from other embryonal tumors. Therefore, screening for this or similar NUTM1 rearrangements should be considered for all patients with unclassified central nervous system tumors with rhabdoid features to ensure accurate diagnosis. Ultimately, with additional cases, we may be able to better inform therapeutic management for these patients. © 2023 The Pathological Society of Great Britain and Ireland.

Advances in optical trapping design principles have led to tremendous progress in manipulating nanoparticles (NPs) with diverse functionalities in different environments using bulky systems. However, efficient control and manipulation of NPs in harsh environments require a careful design of contactless optical tweezers. Here, we propose a simple design of a fibered optical probe allowing the trapping of dielectric NP as well as a transfer of the angular momentum of light to the NP inducing its mechanical rotation. A polarization conversion from linearly-polarized incident guided to circularly transmitted beam is provoked geometrically by breaking the cylindrical symmetry of a coaxial nano-aperture that is engraved at the apex of a tapered metal coated optical fiber. Numerical simulations show that this simple geometry tip allows powerful light transmission together with efficient polarization conversion. This guarantees very stable trapping of quasi spherical NPs in a non-contact regime as well as potentially very tunable and reversible rotation frequencies in both directions (up to 45 Hz in water and 5.3 MHz in air for 10 mW injected power in the fiber). This type of fiber probe opens the way to a new generation of miniaturized tools for total manipulation (trapping, sorting, spinning) of NPs.

Quinoa (Chenopodium quinoa Willd.) is attracting worldwide attention due to its nutritional and biological properties. Nowadays, this pseudocereal is cultivated worldwide in different environmental conditions. This work evaluates the nutritional profile, polyphenol content and antioxidant capacity of five quinoa cultivars (Negra Collana, Chullpi Real, Salcedo Inia, Pasankalla and Kancolla) from Spain and from the Andean region, looking for the key factor of quinoa composition. Nutritional profile was similar for the same cultivar among the locations but, protein and iron contents were higher (p < 0.05) in Spanish seeds compared to the Andean ones. PCA and Pearson correlation coefficient reveal that the darkest quinoa cultivars, Negra and Pasankalla, had the best bioactive profile because the greater dietary fiber, polyphenol content, and antioxidant capacity (p < 0.05), regardless of origin zone. Concluding, the genetic variability seems to have a higher influence than the geographic factors on the nutritional and antioxidant composition of quinoa.

BACKGROUND: Recurrent Aphtous Stomatitis (RAS) is the most common process affecting the oral mucosa. It is painful, multifactorial and generally recurrent. The aim of this systematic review is to know the last treatment approaches and their effectivity. MATERIAL AND METHODS: we compared the outcome of different kind of treatments in terms of the improvement of the lesions, reduction of the size of those lesions and the time needed for their healing. Inclusion criteria were: clinical trials, articles written in English or Spanish and published less than 5 years ago. RESULTS: we used the following keywords: "treatment", "aphtous stomatitis", "canker sores"; combined with Boolean operators AND y OR. We selected 28 articles for reading the whole text, and after applying the eligibility criteria, we selected 17 articles for our revision. Among all the treatments, we emphasize the barrier method based in compound of cellulose rubber and a calcium/sodium copolymer PVM/MA, with which the difference in the 3rd and 7th day was of -6,29 ± 0,14 points in the pain score. The treatment with insulin and chitosan gel, brought a pain suppression on the third day, with no reactivation of the pain during the whole study. The application of a film composed of polyurethane and sesame oil with chitosan, brought a reduction in the size of the lesions of 4,54 ± 2,84mm on the 6th day compared with the situation before the beginning of the treatment. The different kinds of laser, which produced a reduction in the pain score just at the beginning of the treatment up to 8,1 ± 1,6 points, and a reduction of the size of the lesions of 4,42 ± 1,02mm on the 7th day. CONCLUSIONS: Besides the classic treatments for RAS, we have to take into account other treatment modalities, above all the different kinds of laser.

OBJECTIVE: The objective is to determine the prevalence of potentially inappropriate drugs according to the Marc, STOPP, and PRISCUS lists in elderly HIV patients. METHODS: It was an observational, retrospective, and multicenter study. People living with HIV 65 years or older who underwent chronic concomitant treatment were included. Descriptive and multivariate analyzes were performed to study the association between polypharmacy and potentially inappropriate medication compliance. RESULTS: A total of 55 patients were included, 81.8% men and a median age of 69 years (IQR: 67-73). The median number of comorbidities was 3 (IQR: 2-5) and the most frequent pattern of multimorbidity was cardiometabolic (62.9%). The predominant antiretroviral treatment was triple therapy (65.5%). Polypharmacy was present in 70.9% of the patients and 25.5% had major polypharmacy. The most frequent polypharmacy pattern was cardiovascular (69.2%). The percentage of potentially inappropriate medications according to the Marc, STOPP and PRISCUS lists was 65.5%, 30.9% and 14.5%, respectively (p<0.001). Adjusted for age and sex, polypharmacy was not independently associated with potentially inappropriate medication compliance in any of the lists. CONCLUSIONS: Polypharmacy and potentially inappropriate medications have a high prevalence. There is great variability in the percentage according to the list applied. Age, sex, and presence of polypharmacy are not predisposing factors to the presence of potentially inappropriate medications.

Introducción: La disección de los ganglios linfáticos inguinales mediante abordaje videoendoscópico (LIVE) se ha planteado como una alternativa óptima para disminuir la morbilidad y mantener un control oncológico equivalente al del abordaje convencional. Uno de los pasos de mayor complejidad de la técnica es la elaboración de un plano de disección adecuado y entre sus ventajas está una exposición mejorada del campo quirúrgico. El objetivo de este estudio es determinar la viabilidad, en términos de seguridad y control oncológico, de una modificación de la técnica LIVE convencional, utilizando un plano de disección suprafascial, en una serie inicial de pacientes.Pacientes y métodos: Se llevó a cabo una revisión retrospectiva en un único centro de los pacientes tratados con LIVE mediante nuestra modificación de abordaje suprafascial. Se proporciona una descripción de la técnica quirúrgica paso a paso. Los datos recogidos incluyeron las características demográficas y de la enfermedad, las comorbilidades y los factores intra- y postoperatorios.Resultados: Se incluyó a un total de 7 pacientes (12 procedimientos). La edad media era de 64,42 años y el 71,42% de los pacientes eran hombres. El procedimiento se realizó de forma bilateral en el 71,4% de los casos. La mediana de pérdida de sangre estimada fue de 10mL (rango 10-25). El tiempo quirúrgico medio fue de 115 min (rango 120-170). La mediana del número de ganglios linfáticos obtenidos fue de 10,25 ganglios (rango 7-11). Solo un paciente (8,3%) experimentó una complicación de grado iiia de Clavien-Dindo, lo que requirió recambio del drenaje percutáneo. La mediana de tiempo hasta la retirada del drenaje fue de 13 días (rango 10-16). La mediana de duración de la estancia hospitalaria fue de 48 h, oscilando entre 24 y 96 h en toda la serie. (AU)


Introduction: The videoendoscopic approach to inguinal lymph node dissection (VEIL) has been suggested as an excellent alternative carrying lower morbidity but equivalent oncological control. One of its most difficult steps is the development of an appropriate plane of dissection. A suprafascial approach would allow for better exposure. The objective of this study is to determine the feasibility, in terms of safety and oncological control, of a technical modification to the classical VEIL technique using a suprafascial plane of dissection in an initial series of patients.Patients and methods: A single institution retrospective review of patients undergoing VEIL technique using our modified suprafascial approach was conducted. A step-by-step surgical description is provided. Data collected included demographics; comorbid conditions; disease characteristics; intraoperative factors; and postoperative factors.Results: A total of 7 patients (12 procedures) were included. Average age was 64.42 years old, and 71.42% of the patients were male. The procedure was performed bilaterally in 71.4% of the cases. Median estimated blood loss was 10mL (range 10-25). Mean operative time was 115 minutes (range 120-170). Median number of nodes removed was 10.25 (range 7-11). Only one of the patients (8.3%) experienced a Clavien-Dindo grade IIIa complication requiring delayed percutaneous drainage replacement. Median time until drainage removal was 13 days (range 10-16). Median length of stay was 48hours but ranged from 24-96hours within the series.Conclusions: The modified Video Endoscopic Inguinal Lymphadenectomy (VEIL) technique using a suprafascial plane of the dissection, allows a bilateral ILND in highly competitive operative times and with limited morbidity, without compromising its oncological efficacy. Objetivos: Para valorar la viabilidad y precisión de la técnica de SNOLL en la localización de microcalcificaciones mamarias malignas planteamos un estudio prospectivo observacional multicéntrico. Evaluamos la aplicación de la técnica y la evolución de las pacientes en un periodo de 10 años. Materiales y métodos: Entre 2007 y 2010, se reclutaron 64 pacientes. Todas las pacientes habían sido diagnosticadas de neoplasia de mama por microcalcificaciones en mamografía y fueron tratadas con cirugía conservadora y biopsia de ganglio centinela. Las inyecciones de tecnecio-99m se realizaron de forma preoperatoria, con una o varias punciones (en lesiones > 3,5 cm), guiadas por mamografía o ecografía. En todos los casos se realizó una linfogammagrafía preoperatoria para valorar la migración del radiotrazador y una mamografía intraoperatoria para valorar la correcta extirpación de la lesión. Resultados: En las 64 pacientes se realizó la inyección de radiotrazador y la linfogammagrafía demostró la correcta migración del contraste al ganglio centinela, permitiendo la exéresis de la lesión mamaria y del ganglio centinela en el 100% de los casos. La mamografía intraoperatoria mostró la exéresis completa de la lesión en el 100% de los casos. La anatomía patológica mostró radicalidad en la primera cirugía en el 84,4% y supervivencia libre de enfermedad a los 10 años en el 84%. Conclusión: La técnica SNOLL es una técnica factible y precisa para la localización y tratamiento quirúrgico de microcalcificaciones malignas con supervivencia libre de enfermedad y porcentaje de recidiva comparable a otras técnicas. Asimismo, proponemos el afeitado sistemático de la cavidad para reducir la necesidad de reintervención.(AU)


Objectives: To evaluate the feasibility and accuracy of the ROLL technique to localize malignant mammary microcalcifications and the risk of local recurrence, we performed a multicentre prospective observational study to assess the application of the technique and progress of patients with a 10year follow-up. Materials and methods: Between 2007 and 2010, 64 patients were recruited. All the patients had been diagnosed with breast neoplasm due to microcalcifications on mammography and were treated with conservative surgery and sentinel lymph node biopsy. A technetium-99m injection was performed preoperatively, with a single or several punctures (> 3.5 cm lesions), guided by mammography or by ultrasound. In all cases, we performed a preoperative lymphoscintigraphy, to assess contrast migration, and intraoperative mammogram to assess correct removal of the lesion. Results: In the 64 patients, contrast injection could be confirmed, and lymphoscintigraphy proved correct migration of the contrast to the sentinel node, allowing excision of the mammary lesion and of the sentinel lymph node in 100%. The intraoperative mammogram showed complete lesion excision in 100% cases. The pathology showed radicality in the first surgery of 84.4% and showed a disease-free survival of 84% at 10 years. Conclusión: The SNOLL technique (ROLL + SNB) is a feasible and accurate technique for the localization and surgical treatment of malignant microcalcifications with disease-free survival and a relapse rate comparable to the rest of the techniques. Likewise, we propose systematic cavity shave to reduce the need for reinterventions.(AU)
